CSI understands clients and staff have been impacted by trauma. CSI is becoming a trauma-informed organization by hiring individuals with existing experience in trauma-informed practices as well as supporting ongoing staff development with trauma-informed practices. This will include learning to recognize symptoms of trauma and how to respond, through training starting at orientation and throughout their career at CSI. CSI is committed to systematically evaluating the integration of the trauma-informed values of safety, trustworthiness, choice, collaboration, and empowerment at all levels of the organization's policies, procedures, and structures to guard against the possibility of re-traumatization.


Program Overview

Community Solutions Inc is seeking a Full-Time Male Case Manager to join our team at our Torrington AIC program. The AIC, or Alternative in the Community program is a community based alternative to incarceration and provides multifaceted intervention services for offenders awaiting trial, or who are on probation or parole. The program uses Motivational Interviewing techniques and curricula-drive, evidence-based models designed to assess the needs of each client to reduce individual risk factors. As a Case Manager, your role will be to facilitate general case management duties including intake, assessment, case record management, and discharge preparation.


Position Overview

  • Within 5 days of referral collect all intake information and enter into CSI and CSSD data collection systems on the day the data is obtained.
  • Perform client needs assessments within 5 days of intake and enter data into CSI and CSSD data collection systems on the day the data is obtained.
  • Initiate clients into group within 15 business days from the date of referral.
  • Actively participate in Service Team Meetings by presenting new client assessments, clients nearing completion of service, any clients with poor attendance or participation and presenting any new cycles that clients can be placed in. Provide and elicit peer support for client issues as needed.
  • Develop and document individual service plan goals with clients within 5 days of intake and review with client on a bi-weekly basis. Monitor treatment stipulations, job readiness preparation and community resource referrals.
  • Schedule, conduct, and document individual counseling sessions that address client personal issues and program performance. Enter each session into CSI and CSSD data collection system on the day the session is held.
  • Maintain client files and ensure compliance with Agency policy regarding confidential information. This confidentiality must be maintained even if you are no longer an employee of CSI.
  • Submit pre-trial progress reports to your supervisor for review 4 days prior to the court date. All pre-trial reports are to be submitted to the court 2 days prior to the court date.
  • Submit Monthly progress reports for probation clients to your supervisor for review 4 days before date due. All progress reports are due to probation by the 10th of each month.
  • Communicate client non-compliance to the referral source within 24 hours. Document communication in CSI data collection system.
  • Complete incident reports as needed and submit to PD prior to leaving shift.
  • Record all sessions with clients and follow procedure to upload these recordings for quality assurance review.
  • Actively participate in quality assurance review session and apply new strategies and techniques presented.
  • Register for and attend all required trainings and boosters including but not limited to MI, Case Management and Assessments.
  • Provide coverage of front desk as needed and perform the associated duties.
  • Participate in the administration and collection of WAI surveys.
  • Contribute towards meeting all benchmarks on CSSD's Risk Reduction Indicators.
  • Secure urines from same sex clients, complete all required paper work and electronic documentation in CSI and CSSD data collection systems on the day the data is obtained.
  • Monitor case load to ensure all urines and breathalyzes are randomly requested and are secured.
  • May be required to perform tasks carried out by the Intervention Specialist on occasion as directed.
  • Serve as a role model to clients and staff by modeling pro-social behaviors.
  • Maintain a working knowledge of and abide by the Agency's programs, policies, procedures and code of ethics.
  • Perform other tasks as assigned by the supervisor.

Starting wage: $22.10 per hour

Schedules Available:

  • Mon/Wed 12pm-8pm Tues/Thur/Fri 9am-5pm

Qualifications

  • Bachelor's degree and one year experience in case management or similar field. Education may be substituted with experience on a year for year basis.
  • Due to contract requirements a male candidate is needed for this position.
  • Excellent oral, written and interpersonal skills.
  • Ability to engage and develop collaborations with community agencies.
  • Previous assessment experience preferred
  • Experience with Microsoft Office.
